This service provides essential in-home services to keep older adults living safely at home. A care plan is developed that provides assistance with personal care (bathing, dressing, etc.) for the older adult, and/or in home respite for the caregiver as needed. Requests for this service begin with an in-home assessment to determine eligibility and develop an individualized plan of care based on the level of support needed.
*Hours are assigned appropriately to meet the individual support needed
*Basic housekeeping tasks are only available as part of an overall care plan that supports essential tasks.
Status: Due to staffing challenges, eligible older adults may experience a wait until staffing has been identified.
